By Mara Buchbinder

Even though soreness is a common human adventure, many view the soreness of others as deepest, immune to language, and, hence, basically unknowable. And, but, regardless of the most obvious limits to comprehending another’s inner kingdom, language is all that we have got to translate soreness from the solitary and unknowable to a phenomenon richly defined in literature, medication, and way of life. with out denying the personal dimensions of soreness, All on your Head deals a wholly clean point of view that considers how soreness can be configured, controlled, defined, or even skilled in deeply relational ways.

Drawing on ethnographic fieldwork in a pediatric soreness sanatorium in California, Mara Buchbinder explores how clinicians, adolescent sufferers, and their households make feel of confusing signs and paintings to relieve ache. via cautious cognizance to the language of pain—including narratives, conversations, types, and metaphors—and distinctive research of the way younger discomfort victims make which means via interactions with others, her e-book finds that even though inner most discomfort will be, making feel of it really is profoundly social.

Taking the funnel as a point of departure, I explore the metaphors that pediatric pain practitioners use to explain chronic pain to adolescents and families. Having established that metaphors are a key element of clinical rhetoric, I turn to the question of how they persuade by examining how explanations of pain take shape within the clinical encounter. In chapter 2, the first of two chapters that focus on the neurobiological explanatory frame, I examine a clinical explanatory model that linked the neurobiology of chronic pain to certain qualities of smart, talented, and virtuous adolescents.

Most West Clinic patients found their way to the clinic after consulting at least three specialists—typically, gastroenterologists, neurologists, orthopedists, and rheumatologists. 4 One mother referred to this thwarted quest as a journey through the “Bermuda Triangle” of pain, a metaphor that captures the aimlessness and sense of abandonment that many families experience as they search for help. Some disappear into the medical system, feeling rudderless and bewildered. Mark Siegel’s pathway to the West Clinic exemplifies the circuitous routes that many families take before finding their way to pediatric pain clinics.

As a result, and also because multiple family members accompanied some patients to the clinic, the examination rooms were often crowded. I practiced slinking into narrow corners and leaning against walls unobtrusively, trying not to get in the way. I did carry a clipboard and take notes, but this did not make me stand out, since most of the others in the room (including the families) were also taking notes. Introduction | 29 During patient consultations, I usually remained silent, but if the physician (and his or her entourage) left the room to talk about the treatment plan or to retrieve a prescription pad, I would often remain behind to talk to the family.
